NEXT STEP(Required)

You will receive an email after filling out this form with your PERMANENT Club Rush number and instructions on how to pay for it. You are not in the draw until your number is paid for. NOTE: If you are filling this out on a weekend or holiday, you will receive the email the next business day. Club Rush is drawn every Friday by noon, the payment cut-off to be entered that week is 10 AM that Friday. Any amounts received after that cut-off will be entered in for the next week's draw.
